Terms & Conditions for Laser Tattoo Removal in Tamworth, near Lichfield, Laser Therapy for Pain Relief in Tamworth, near Lichfield, Fungal Nail Laser Treatment in Tamworth, near Lichfield, Laser Therapy for Acne and skin conditions in Tamworth

Governing Terms and Conditions. The terms and conditions set forth in this Contract shall constitute the sole and exclusive agreement between Fadewell Solutions LTD, trading as "Sunstone Laser Pain Clinic/CAM Therapy" and the Customer.


HEALTHCARE
1.    I understand that Fadewell Solutions Therapists, Laser Technicians and Complimentary Therapists are not medical doctors and do not diagnose illness, disease, or any physical or mental disorder. I acknowledge that laser therapy, shockwave therapy, ultrasound therapy and massage therapy are not substitutes for medical treatment, and that Fadewell Solutions, “the company”, recommends I see a primary healthcare provider for that service. I understand that it is my responsibility to communicate with my therapist if I have concerns or questions about my session. I do not have any injuries or conditions that would prevent me from receiving laser therapy, shockwave therapy, ultrasound therapy and massage therapy nor have I been told by a health care provider that I should not receive alternative therapies.


COMMUNICATIONS
2.    Any information exchanged during treatment or body work session is confidential and is only used to provide me with the best health care services available. I understand that a therapist will ask me questions about my health and physical condition and that I am obligated to answer truthfully and honestly about my health history in full detail. 
3.    I understand that my feedback is essential in my treatment, and that if I experience any unusual discomfort and/or pain during my treatment session, it is my responsibility to inform the therapist in order to enable the therapist to adjust the pressure or technique being used. 


TREATMENTS
4.    The therapist reserves the right to decline, discontinue, or restrict services based on any provided information that may indicate that the treatment would put my health or the therapist’s health at risk. While all materials used in the process of laser treatments are clean, please note that in some people infection through poor aftercare is possible (laser tattoo removal/Fungal Nail Laser treatment). Reaction and dryness can occur and this is not the fault of the laser specialist. Your laser specialist will guide you through aftercare methods. 5.    The practitioner/therapist does not prescribe medical treatment of pharmaceuticals, nor does he/she perform any spinal manipulations.
6.    I understand that service offered today, and in the future, are not a substitute for mainstream medical care, but are “complimentary” “non-mainstream health approaches used alongside conventional medical treatments to support well-being, manage symptoms, and improve quality of life”, rather than replace standard care and that any information provided to me by the therapist is purely for educational purposes and is not diagnostically prescriptive in nature. Fadewell Solutions Clinic is a “treatment clinic” and not a diagnostic clinic.
7.    I acknowledge and understand that pain relief varies person to person because of individual biological differences (genetics affecting receptors) and psychological factors (placebo effect), ref. The British Pain Society
8.    I have stated all of my known medical conditions on the Client Intake form. I have consulted a medical doctor or licensed medical health care practitioner regarding any checked or described conditions.
9.    I understand that it is solely my responsibility to keep the therapist updated on any changes in my physical health and I further understand that the company and the therapist shall not be liable for any purpose and for any reason whatsoever, should I fail to do the needful as per this paragraph.

APPOINTMENTS
10.   I acknowledge that I am responsible to be on time for my appointments and that the therapist is not under any obligation to extend my therapy session. I also agree that I am responsible to pay for the full time I have booked with the therapist if I am late. I understand that my appointment time is reserved for me only. It is not acceptable to show up late and ask for a reduction in price in proportion to the reduction in time. NOTE: The appointment is for “both parties” and we will book out X number of minutes of our time to you. If you’re late for your appointment then there may be a considerable waiting time to be seen or you may be allocated another appointment slot due to other client visits. We will not keep other customers waiting who have arrived on time for their appointment. If I miss an appointment or am unable to give twenty-four (24) hours’ notice when I need to change or cancel my appointment, I agree to pay the company in full for the booked appointment time. The patch test is a fixed fee [Tattoo Removal]. This is off-set against the cost of your first treatment, If you are not suitable to continue having laser or choose to not have laser for any reason, under no circumstances is this patch test fee refunded. This fee covers the use of the room, the use of the machine and your practitioners time and expertise. Making a booking when you are not able to have the treatment and didn't check with us beforehand (under 18, pregnant, breastfeeding, some medical conditions or medications) will mean that your booking fee either goes towards a treatment that you can have, or held until you are in a position to have the treatment, or would mean being forfeited. Clients are expected to do their research and check with us beforehand if they are any of the above, or aren't sure, before booking.

CONDUCT 
11.    The care of our clients is our top priority. Therefore, visitors must conduct themselves in a respectful way. If you act in a way that is abusive or untoward you will be asked to leave the premises. If this is via message you will be denied entry in future. At any time if any abuse or negative behaviour has occurred, no refunds will be give, we reserve the right to refuse custom. Any deviation from the for mentioned will result in the termination of your booking and your removal from the premises. Any slanderous or unconfirmed reviews on any public domain will be investigated and legal action may be taken.


GDPR
12.    EU General Data Protection Regulation. Privacy notice 2018. May 2018.We are required by law to document what personal information we keep for all clients. At the time of booking your appointment your full name and contact details will be taken.  At your appointment further contact details, a medical history and consent will be recorded as required by law. These records are kept private and confidential. You have the right to request to see your forms at any time. We do not send marketing emails or letters however we aim to adhere to the GDPR rules and changes. We do not pass on any information to any other third parties or use the information for any other way other than relating to your cosmetic treatments. We legally have to keep your information (address, contact details, what treatment you had on what date, and also photographs for legal reasons) for 5 years. Failure to do this results in a void in insurance.

 

ACKNOWLEDGEMENT
13.    I have reviewed this form in its entirety and I have discussed all my concerns regarding my treatment with my therapist. We are happy to advise on any issues that may arise, and we recommend that you contact us straight away if you have any concerns. Proof of age will sometimes be requested so always have it with you for your appointment.
